    Description:
    The Connecticut Long Term Care Ombudsman Program is convening a Regional RCH Community Integration Roundtable to bring together Residential Care Home providers, Access Agency staff, and community partners for a focused, collaborative discussion on strengthening community-based supports and transitions.

    These roundtables are designed to build stronger connections across systems, deepen shared understanding of available services, and improve coordination when individuals in Residential Care Homes begin exploring community-based options.

    Participants can expect a practical, conversation-driven session that includes:

    • A clearer understanding of Medicaid waiver programs and services available to individuals residing in Residential Care Homes, including how and when to initiate referrals
    • A broader perspective on Residential Care Homes as a critical service delivery system within the long-term services and supports continuum
    • Discussion of real-world, complex cases, including strategies to support individuals in remaining in their communities of origin whenever possible
      Opportunities to problem-solve common barriers to transition, coordination, and service access
    • Direct engagement with Access Agency staff, waiver case managers, and community partners to better understand roles and processes
      Time to network, build relationships, and establish ongoing points of contact across organizations
    • This is a conversation-based session grounded in collaboration, practical application, and shared learning. The goal is not formal training, but rather strengthening the relationships and understanding necessary to better support individuals in making informed choices and achieving successful community integration.
